The table or stand is mine, and you can get it here.

You should be able to cut a square box lid and just place it over the top of the stand.
The inner boxes in this post are scalloped and should work when sized correctly.
This post has a lid with a Top Note styled cut. Either use black-out or close/delete the layer with the cut-out so that the lid is solid. HTH
