Folding gift tag for bag,

Hi all,
Not sure how to put this but here it goes. First thanks to all of you for helping out those of us who are SVG challenged. I have to make some gift tags for wedding gift bags. I need tags that fold in the center and have a hole to put ribbon through. I found one here shaped like a bird, double sided, hole on both sides but I do not want a bird shape, I would like a square approximately 2 inches (after folding) and maybe a little bubble, bump, knotch on one corner for the hole to thread the ribbon through. I would like it to open left to right. Anyone out there that can help me out? Thanks in advance for reading this
